What is a DOT physical?

A DOT physical — officially the FMCSA medical examination — is a federally required health check for anyone who operates a commercial motor vehicle. It exists for one reason: to confirm that a driver is medically able to handle the demands of the road safely, for themselves and for everyone around them. The exam must be performed by a medical examiner listed on the FMCSA National Registry, which is exactly what we are.

A passing result produces a Medical Examiner’s Certificate — the “DOT card” you carry to prove you are cleared to drive. That card is valid for up to 24 months. Because a driver’s livelihood depends on it, we treat the exam as more than a checkbox: we explain each step, answer your questions in English or Spanish, and make sure you leave understanding your result and any next steps.

Who it is for

Who needs a DOT physical?

You need a current DOT medical card if you drive a commercial motor vehicle that requires a CDL, cross state lines for work, carry certain passenger counts, or haul placarded hazardous materials. In practice that covers most CDL-A and CDL-B drivers, owner-operators, bus and shuttle drivers, and many delivery and construction drivers.

What is covered

What the exam includes

Every DOT physical follows the same FMCSA checklist. Here is what happens during your visit, start to finish.

Vision

Distance acuity and peripheral vision, with a standard of at least 20/40 in each eye (corrected is fine). Bring your glasses or contacts.

Hearing

A forced-whisper test or audiometry. You can pass wearing a hearing aid, so bring it if you use one.

Blood pressure & pulse

You need a reading below 140/90. We re-check calmly if nerves push the first number up.

Urinalysis

A quick urine test that screens for underlying conditions like diabetes or kidney issues. It is not a drug test.

Health history

A short, plain-language review of your medications and any conditions such as diabetes, heart issues or sleep apnea.

Physical exam

A hands-on check of your heart, lungs, abdomen, spine, limbs and basic neurological function.

Come prepared

What to bring

Bringing the right items keeps your visit to about 30 minutes and helps you avoid a second trip.

  • A valid photo ID (driver license)
  • Your glasses or contact lenses
  • Hearing aids, if you use them
  • A current list of medications and dosages
  • Paperwork for any managed condition — diabetes, sleep apnea, high blood pressure or heart conditions
  • Your CDL or CLP number, if you have one
Nervous about blood pressure? The white-coat effect is real. We let you settle and re-check during your visit, so an anxious first reading will not automatically cost you your card.
Good to know

How long is your medical card valid?

Most drivers who pass are certified for the full 24 months. The examiner can issue a shorter certification — for example 3, 6 or 12 months — when a condition needs monitoring, such as blood pressure that is being managed with medication. If that happens, we explain exactly why and what you can do to earn a longer card next time. Keep your certificate somewhere safe and note the expiration date; renewing before it lapses keeps you legal and on the road without interruption.
